Case Management

WorkWell has the goal of delivering the highest quality of care to an injured employee without unnecessary or unneeded care. With this goal in mind WorkWell develops treatment pathways for patients in both employer and non-employer directed states.

WorkWell's acute intervention program manages injuries from the moment they enter our system. WorkWell can be notified of injures by phone, fax, employer web portal or email. Once the information has been entered into our system our trained intake personnel coordinate the initial appointment for injured workers making sure all special instructions, including preferred providers, in our custom developed database are followed. Intake coordinators verify all information to make sure it is correct including patient demographics, employer name and location, insurer, date of injury, injury type and mechanism of injury. Once this critical information has been verified and the appointment has been scheduled the case is transfer to WorkWell's case management department.

Our proven approach relies on two key components: information management and treatment management. These components fit together seamlessly to deliver significant savings and dramatically reduced lost work time. All management must happen at the right time, quickly and accurately for it to be effective.

WorkWell case managers schedule all appointments, monitor treatment and intervene when treatment goals are not being met. Case manager use our internal proprietary guidelines to assist them in managing each case. These guidelines are based on over 25 years of clinical data specific to workers compensation.

It is imperative that all relevant parties are kept abreast of all changes in the care of an injured employee and WorkWell's information management is the key to staying on top of each case's status. After every appointment with a WellNet provider, WorkWell case managers collect records and immediately release patient status reports to all necessary parties. This communication assures that all relevant parties aware of the initial diagnosis, follow-up care and expected return to work date. The WorkWell Medical Report Form and Patient Status Report are standardized, providing clear and consistent information delivered electronically as soon as they are available.

Also, collected records are retained to provide and accurate record of the patients progress throughout treatment. Reports can be run from this website on many differ aspects of treatment such as PT utilization, updated status reports and upcoming appointments. Keeping you informed about the where, when and why of injuries is critical to overall success.
